S2:E32 Dementia Care Tips for First Responders

Are you aware that First Responders are Amazing! Police, Fire, EMS and More, all wired to run towards danger for the greater good of people. It is a challenging career with ever changing daily activities, responsibilities and experiences that range from routine to out of this world. Many of you may have great experience caring for individuals with memory loss but I too thought I understood dementia only to get rudely awakened to my inexperience when my own mother threw me out of the house. I was surprised and hurt by her unexpected anger but determined that day to find out why people living with Dementia do what they do!
If we start with some pictures of possible emergency scenarios that you might encounter, many of you can immediately explain what you do next. There is a car on fire, a man holding his chest or the very pregnant woman who appears to be in great distress. Your training and experience will direct you on what to do next just like mine did when I worked in the level three Trauma Center in my twenties. I don't claim to know your job but I knew what to do once you brought them to me at the Trauma Center.
The challenge comes with the picture of the woman who doesn't seem to have any problems at all. She appears to be a sweet elderly woman with a grandbaby on her lap, but let's look a little closer. Can you see the Dementia? No? Well she actually is mid stage with her journey with dementia and can become very demanding if you push the wrong buttons. That is where we struggle. We need to be better detectives with skills to join them on the journey and make sure we are not part of the problem when working with these individuals.
Next, we will review some of the more common symptoms of Dementia and how you can respond. Probably the most important tip I can give you today is to be aware of Anosognosia. This is a term that changed everything about my care for my Mom and I need to credit Teepa Snow with Positive Approach to Care for this education. Anosognosia is the Inability for a person to see their own brain deficit. We described how it affects people living with BiPolar Disorder or Schizophrenia but the greatest revelation for me was learning it can develop in 50% of people living with a Dementia.
It is important to understand that some people living with dementia have awareness of their diagnosis and are usually sad, tearful and maybe even fearful about their disease. Others with Anosognosia have no awareness of this problem and as a matter of fact it is always your problem. They are paranoid, suspicious, never wrong, always right but always the victim. These can be a no win situation without training and this is when they call for help from First Responders or from me to sort out why they do what they do.
We are dealing with a person with no logic so no amount of explaining will suffice. Hit the Pause Button and realize that what works is to Validate their position, De-Escalate the energy in the room THEN we can redirect them emotionally or physically to a happy place. We need to allow them to think they are right even when they are wrong and ask them to help us if possible to get them to do something. Sounds counterintuitive but it works.

S2:E32 Dementia Care Tips for First Responders

Hey Care Partners,
Are you aware that First Responders are Amazing! Police, Fire, EMS and More, all wired to run towards danger for the greater good of people. It is a challenging career with ever changing daily activities, responsibilities and experiences that range from routine to out of this world. Many of you may have great experience caring for individuals with memory loss but I too thought I understood dementia only to get rudely awakened to my inexperience when my own mother threw me out of the house. I was surprised and hurt by her unexpected anger but determined that day to find out why people living with Dementia do what they do!
If we start with some pictures of possible emergency scenarios that you might encounter, many of you can immediately explain what you do next. There is a car on fire, a man holding his chest or the very pregnant woman who appears to be in great distress. Your training and experience will direct you on what to do next just like mine did when I worked in the level three Trauma Center in my twenties. I don't claim to know your job but I knew what to do once you brought them to me at the Trauma Center.
The challenge comes with the picture of the woman who doesn't seem to have any problems at all. She appears to be a sweet elderly woman with a grandbaby on her lap, but let's look a little closer. Can you see the Dementia? No? Well she actually is mid stage with her journey with dementia and can become very demanding if you push the wrong buttons. That is where we struggle. We need to be better detectives with skills to join them on the journey and make sure we are not part of the problem when working with these individuals.
Next, we will review some of the more common symptoms of Dementia and how you can respond. Probably the most important tip I can give you today is to be aware of Anosognosia. This is a term that changed everything about my care for my Mom and I need to credit Teepa Snow with Positive Approach to Care for this education. Anosognosia is the Inability for a person to see their own brain deficit. We described how it affects people living with BiPolar Disorder or Schizophrenia but the greatest revelation for me was learning it can develop in 50% of people living with a Dementia.
It is important to understand that some people living with dementia have awareness of their diagnosis and are usually sad, tearful and maybe even fearful about their disease. Others with Anosognosia have no awareness of this problem and as a matter of fact it is always your problem. They are paranoid, suspicious, never wrong, always right but always the victim. These can be a no win situation without training and this is when they call for help from First Responders or from me to sort out why they do what they do.
We are dealing with a person with no logic so no amount of explaining will suffice. Hit the Pause Button and realize that what works is to Validate their position, De-Escalate the energy in the room THEN we can redirect them emotionally or physically to a happy place. We need to allow them to think they are right even when they are wrong and ask them to help us if possible to get them to do something. Sounds counterintuitive but it works.
Better Ways to Communicate When You Recognize Anosognosia:
Always approach from the front, imagine they are wearing a scuba mask as most people with Dementia lose peripheral vision.
Don't Rush them and use fewer words.
Extend a Hand and Ask them for Help.

Let's review some important tips:

Have you ever considered how Improv can work with people living with Dementia. Tom Badger has years of experience teaching improv and he is a huge volunteer with the local Alzheimer's Association.

I have been a Support Group Facilitator for the Alzheimer's Association for 5 years. I have also done some extensive public speaking for the Alzheimer's Association over the years.
I facilitate a group in Lakeside Park, KY and I also now facilitate a second support group in Goshen OH. I have some really strong and passionate opinions on how families can better deal with the challenges that they face in caring for a loved one with dementia.
He believes families need to REALLY unite together to help their loved one - make sure that not just one person is doing all the work - because in my opinion, that is often a recipe for disaster.
It is very important for the primary caregiver to recruit AND educate their friends and family so they don't end up doing this alone. If they do NOT ask for help, and if they are NOT specific about the help they need - they often end up doing it themselves.
Tom feels the main thing that family and friends need to learn when they have a loved one with dementia is that "your reality" doesn't matter much to them anymore. The disease has robbed the loved one with the ability to do SO MANY THINGS, and one of those things is the ability to show feelings for others, listen effectively to others, and show true empathy to others.
What is now MOST IMPORTANT to them is their own reality - we will call that their "Base Reality" The sooner that caregivers and family members can learn to "live in the loved ones base reality" - the better everyone will be!
The parallels of improv and caring for a patient with Alzheimer's or another dementia are many! At its core, communication with an Alz. patient is best when there is no arguing, no chastising, very little negative talk. The best improv scenes are when two or more people are agreeing with the other person's thoughts and feelings, they are advancing the scene with the famous phrase "Yes, AND" - and the less arguing the better for both the improvisors and the audience!

I hope you enjoy this podcast concerning BEHAVIORS. If you have known me for very long you will know that I don't like the word at all and feel like we misunderstand our loved ones and inappropriately medicate them when they are using cuss words or seem aggressive.
In my experience, I can confirm for you that they are still trying very hard to communicate with you and frequently it is OUR challenge to be a great detective and to figure out what they are experiencing or needing instead of sedating them into submission with antipsychotics. We can be guilty, just because of our inexperience, of creating a monster with over correcting, finger wagging, and trying to control instead of joining them on their journey.

Dimity Orlet has served as the Executive Director of Pro Seniors, a nonprofit that helps older Ohioans prevent and resolve legal and long-term care problems and fight Medicare fraud, since March 2019.
As chief executive officer for Pro Seniors, Ms. Orlet is a passionate aging advocate
and is responsible for overall administration, fundraising, and operation. She serves as the face of the organization, nurturing strategic collaborations and partnerships. Ms. Orlet also leads the management team to ensure the effective provision of services and adherence to regulatory and grant requirements and budgets.
Prior to her work at Pro Seniors, Ms. Orlet served as Assistant Counsel and Director of
Continuing Legal Education for the Cincinnati Bar Association for 18 years. During that time, she cultivated relationships with members, volunteers, and other partners and led the production of 100+ continuing education programs each year through collaboration with practice group leaders and development of strategic partnerships. How are you doing caring for another person? Are you caring for a parent, spouse or sibling? These are challenging seasons in our lives and you have heard me say it before that you will need to bring in help or take advantage of a retirement community. It is just too difficult to go it alone and many of those individuals will get sick and die before the person they are caring for.
Today we have a special organization that I have watched grow over the years. They demonstrate great service to their community with a tremendous number of programs.
It takes an amazing director to make it all work together. Jennifer Harcourt is that amazing Director at Clermont Senior Services, where she has worked for 10 years.
In her current role, she oversees several departments including: transportation, home delivered meals, lifelong learning, adult day services, and the volunteer department.
Jennifer serves on the Executive Committee of the Clermont County Safety Net Alliance and has held many volunteer leadership positions throughout the years.
Jennifer is a Miami University graduate and was born and raised in Clermont County, graduating from Williamsburg High School. She and her husband have 1 son who is currently serving in the United States Army, one daughter, and one granddaughter.
Plan to hear about the wonderful services available and who is eligible for each of them. They serve almost 4000 individuals last year. The county is very large and they manage to do everything from transportation to doctor's appointments and dialysis to recreational visits to Walmart. All of the programs are Monday - Friday and they travel a lot of miles to help.
Then there is the Senior Center itself where they offer multiple activities to respect choices and even have elevated gardening spaces and fishing opportunities onsite. The staff and volunteers are trained to support the seniors in many ways including two meals, ambulating with assistive devices and even toileting.
They also have home safety teams for the homebound and this is a wonderful opportunity for volunteers too. Getting connected is vital so look for the website and Facebook page addresses listed below.

We have a special guest today representing The Giving Voice Foundation. Kristin Cooley is a clinical social worker who serves as the Program Director for the Giving Voice Foundation. In her Program Director role, she is providing leadership as well as strategic direction for planning, implementation, and growth of local programming for caregivers and persons affected by dementia.
She has more than 15 years of experience working with individuals with chronic and/or terminal illnesses (along with their care partners) in both inpatient and outpatient settings. Her professional journey includes 6+ years at the Alzheimer’s
Association, where her eyes were really opened to the uniquely difficult challenges a dementia diagnosis can bring to an individual and their loved ones.
Over the years, Kristin has facilitated numerous caregiver support groups, provided outpatient counseling services, and offered dementia education seminars in professional development settings as well as to the general community.
Kristin’s areas of specialty include but are not limited to: caregivers and/or older adults (specifically impacted by Alzheimer’s and other forms of dementia) as well as grief work, loss of independence, role transitions, isolation, anticipatory loss, and loss to death).
Kristin obtained her undergraduate degree in Psychology from Indiana University and her Master of Social Work from University of Illinois.

Today we will learn about the history of Cannabis and why there is still a stigma. Mary Lynn Mathre, has close to 50 years of experience as a nurse and has specialized in addictions nursing since 1987 and cannabis education since 1990. Ms. Mathre is a co-founder and President of Patients Out of Time (incorporated in 1995) as a non-profit organization devoted to educating health care professionals and the public about the therapeutic uses of cannabis (www.medicalcananbis.com).
Mary Lynn is the editor of Cannabis in Medical Practice: A Legal, Historical and Pharmacological Overview of the Therapeutic Use of Marijuana (1997) and co-editor of Women and Cannabis: Medicine, Science and Sociology (2002).
She has served on the planning committee for Patients Out of Time’s accredited National Clinical Conference on Cannabis Therapeutics conference series since it began in 2000 (now an annual event beginning in 2015). Ms. Mathre is also a founding member and Past President of the American Cannabis Nurses Association (2010). She served as charge nurse on the Addictions Treatment Unit at the University of Virginia Health Center and later as the Addictions Consult Nurse for the UVA Health System until 2002.
In addition, she later worked as the executive director at a methadone treatment center. She has been a past board member of the International Nurses Society on Addictions and serves on the editorial board for the Journal of Addictions Nursing.
She impressed me with her vast experience and the results of her thesis for her Master's Degree. Listen in and you may need to listen again to get all the information shared here. The podcast today came after I was counseling a family who asked me to do a podcast on what do I do after I get a diagnosis. Great question, and I am sure it applies to many.
We are going to mix it up a little today and use not just my information but we will add in three different organizations and their lists for what to do after the diagnosis.
We will refer to A Place for Mom, National Institute on Aging, and a great article from
Tamara McClintock Greenberg, Psy.D., M.S. an associate clinical professor of psychiatry at the University of California, San Francisco.
Do your research and learn about the diagnosis.
Get a medical check up and see the specialist early.
Find a support group for patient and the caregiver.
Assess for awareness of the diagnosis. Some will know they have Dementia and some will not and we may need more training to care for these individuals.
Make the living space safe and assess driving skills.
Find appropriate activities.
Manage the financial, legal and advanced directives.
Know your plan B when the current care situation is no longer appropriate.
Bring help in ahead of the need.
Manage the medication as they decline for example using medisets.
Maybe participate in a clinical trial.
If you live alone with a diagnosis, consider notifying the local EMS and wearing a medical bracelet.
Don't ignore the emotions that come up when caring for someone with Dementia.
The demands of Dementia can tax even the most well meaning caregiver.
It's OK to grieve during this caregiving season.
I hope this fills a need for you on your journey.

Judy Cornish is an author, founder of the Dementia & Alzheimer’s Wellbeing Network (DAWN®), creator of the DAWN Method® and retired elder law attorney.
Her two books (The Dementia Handbook and Dementia With Dignity) take person-centered dementia care from theory to practice by teaching families how to provide strength-based dementia care.
Through DAWN, Judy provides consulting, dementia coaching classes and an online video series for families and home caregivers. Her goal is to help families learn how to truly support their loved ones and enjoy more companionship as they live with dementia.
Listen and watch as we talk about what led Judy down this path and how she developed her unique approach, The DAWN Method. Her methods did not come from caring for a family member but actually started watching a neighbor and realizing the current model did not work but identifying the experiential model was key. Rational thinking tends to go away along with sequencing.

Hello Care Partners, I frequently get asked if we can FIX Dementia. The short answer is NO. As of today, we cannot fix Dementia. There are lots of ways to join them on the journey but if you goal is to Fix them, we definitely need discuss this.
Setting goals as a care partner, are very important and need to realistic and personalized. Everyone is different, and your person's needs will be different and ever changing. Maybe our goals start with: Great Relationships, Decreased Stress and Agitation, Pain Management, Better Nutrition and Hygiene. We also need to incorporate Purpose. This is vital especially in the earlier parts of the journey. There are many examples in this podcast of providing purpose.
The toughest part is the unpredictability of this diagnosis, so we always need to be planning for plan B. This may look like hiring caregivers or extending their hours. It also may be having your name on a waiting list at a community or memory care. This,
just in case, plan will save you and your family hours of worry.
Can we Fix people living with Dementia? NOPE
Can we come alongside with personalized care and love them on their journey? Absolutely. We are here to help.

Self Awareness, when it comes to brain change, is an important topic all caregivers need to understand. Does your person know they have Dementia? My Mom did not know and might have raised a fist if you questioned her on it. I needed to understand what made her react in that way. I was determined to find the answers.
Now, In my experience, I feel it is vital to learn to recognize the difference because the care plans are so very different. Today we are backing up to review some common signs and symptoms of Dementia then discuss the more prevalent diagnosis.
Ultimately, I want you to be a great detective not a judge and be able to identify their awareness and join them on their journey. YOU GOT THIS!!

Since being licensed as a psychologist in 1995, he has worked primarily with older adults. In 2003, he helped found and directed the Centers for Interactive Geriatric Assessment, where he provided geriatric and neuropsychological assessments.In 2016, he formed, with his partner, Babu Gupta, MD, the NeuroPsych Center of Greater Cincinnati. The goal of this Center was to provide comprehensive mental health services to older adults. He continues to provide geriatric assessments and neuropsychological evaluations.Specific areas of focus or expertise include the diagnosis of various forms of dementia and cognitive impairment, behavioral management of dementia, issues surrounding competency, and the functional assessment of older adults.In this Episode, we will discuss the benefits of a neuropsychological evaluation that go far beyond the diagnosis. The colors were selected based on research determining what colors are perceived for the longest as visual perception changes during this disease process. So, you will see a lot of blue, green and peach in this house. Caring for someone with Dementia can be so challenging because people are all different and brain changes progress in all kinds of ways. Your loved one's symptoms will be different than mine and we are dealing with more than 120 diagnosis which are causing the problems in the first place.Today we are doing to look back at several of my "What If" situations to help you find your way when it happens to you. Remember to take a deep breath and pause before you make the situation worse. We can frequently join them on their journey with love and kindness if we can control how we react to each situation.Will there be surprises? Absolutely! Will there be tears of joy and sadness? Yes, that too! Can you survive the challenge ahead of you? Today we are going back to the basics for families and caregivers new to Dementia and Brain Change. It can be challenging to understand why they do what they do and I only wish someone would have been there to pull me aside with some great tips and techniques to keep the peace with my Mom.
Sadly, I thought I understood Dementia because I was a seasoned RN, but soon found out that I was wrong. Mom threw me out of the house and made it clear I needed more training.

Lisa Coors, MBA is the Owner of Coors Core Fitness (CCF) ,the leading medical fitness studio in the Cincinnati area. Started in 2006, CCF has become the referral source for numerous physician practices and physical therapy centers.
Lisa is certified through the American Council on Exercise as a Medical Exercise Specialist and Health Coach. Lisa is also a national presenter in the fitness industry where she presents on chronic disease management and Parkinson’s exercise. She has written numerous publications and is one of the authors for the American Council on Exercise: Medical Exercise Specialist Manual.
She is the creator of the Move it or Lose it for Parkinson Disease Train the Trainer Course, an ACE approved CEC course. She has taught this all over the country to fitness instructors, physical therapists, occupational therapists, caregivers, therapists and patients.
In 2019 she co-Founded Parkinson Community Fitness a non-profit, 501c3 facility that provides a place for Person’s with Parkinson’s to go for exercise, support and social activities. It is located in Blue Ash, Ohio. PCF has grown to over 215 members and continues to grow daily.

Today we are going to dig into the BIG question of Why we don't have a pill to fix Alzheimer's and Dementia. With our special returning guest, Marion Kyner we will discuss what is available for our loved ones on their journey with brain failure.
Marion Kyner, MSN, RN, PMHCNS-BC, CDP, CMC
Marion is an Advanced Practice Nurse and a Psychiatric Clinical Nurse Specialist with over 40 years of experience in nursing. She worked in a Senior Care Community setting for 15 years. She is a certified independent trainer and coach in the Positive Approach to Care model by Teepa Snow. Currently, she has a private practice in dementia care consultation and is an adjunct faculty member at Longwood University Dept. of Nursing in Farmville VA and a clinical instructor at the University of Virginia School of Nursing in Charlottesville, Virginia.
She is passionate about changing the culture and HOW nurses provide care for patients with dementia. She is a community volunteer with the Alzheimer’s Association and Dementia Friends. She has 2 adult daughters, a son-in-law, and a new granddaughter born December 26. She lives with her husband in Cumberland, Virginia.
1. Is there a pill for Alzheimer’s Disease?
a. There is NO pill to cure Alzheimer’s or any other dementia
b. Much like many other Chronic Health conditions like diabetes and heart disease,
or multiple sclerosis, we treat the symptoms.
2. What about the newer drugs recently approved by the FDA?
a. (Leqembi) lacanemab anti-amyloid medications that reduce the amyloid
plaque surrounding the nerve
b. Accelerated approval Jan 2023; full approval July 2023, MCI and Mild Alz Dz
Study was over 18 months – no info on long term effect or safety
Shown to have a moderate effect on slowing decline in memory and
thinking on paper tests, only minimal change in functioning
IV Infusion every 2 weeks at an infusion center
Side Effects: N/V, Chills, body aches, HA, Dizziness, ^ HR, SOB
Serious Side effects: brain swelling; small brain bleeding and death
Talk to doctor about these risks vs benefits
Exclusion criteria: genetic testing for APOE gene, stroke or
TIA, blood thinners, and other health conditions such as RA or uncontrolled diabetes
Imaging tests:– Amyloid PET scan or spinal lumbar puncture measure amyloid
Follow up MRI scans to check for swelling and bleeding
Cost is close to $27,000/ year, 80% Medicare, ? other insurance
Coverage for cost of imaging is uncertain
3. What are the other medications doctors often prescribe
for dementia?
a. Memory Enhancers – cholinesterase inhibitors & Glutamate Regulators
4. Discuss the Psychiatric Medications that are used, like the
new one Rexulti
a. Think of Symptoms Depression, Anxiety, and psychosis (hallucinations & delusions) Seizures and Sleep problems 50% patients with dementia are prescribed an antidepressant
Psychiatric medications were approved for disorders of the brain where there is often a chemical imbalance. Sometimes they work and sometimes they don’t, maybe 30% use
In dementia, we have brain changes and loss of nerves and chemistry – medications may not work the same as they were intended for psychiatric disorders

Hello Care Partners, I started this podcast on my soapbox with a complaint about people posting videos of their loved ones with Dementia on Social Media. I don't know about you but I sure hope my kids don't think this is a good idea. There can be a time and a place for training with video but to repeatedly ask "Do You Remember" questions with a person who has no capacity to answer is upsetting to me. I just needed to vent a little bit because everyone deserves respect and dignity especially when you don't understand you are being used for likes on social media! Exhale.
OK I'll get off of the soapbox. Today we are reviewing questions that are frequently asked and then I will use some posters to bring home some better techniques when caring for someone living with memory loss.
It is important to realize that your person living with Dementia will act differently than mine. That is ok. The goal is to develop a great relationship with them and love them through this journey. I believe in you and I am proud that you are willing to take the time to learn more so you can serve then better.

Hey CarePartners, Identifying symptoms of Dementia is very important but it is even more important to rule out what treatable illness could be causing your loved one to appear to have a type of Dementia. Keep in mind that a true Dementia means parts of your brain are dying. It is progressive and terminal. Conditions that come on quickly usually are not a true dementia and we should see our healthcare provider to diagnose the illness and begin treatments. Ep 33: How to Have Happy Holidays with Dementia

Celebrating family events gets more difficult as our person living with Dementia declines. Challenges like over stimulation or sheltering them from everything can both cause problems. Personalized care is important and we need to recognize this chronic debilitating disease is unpredictable and will affect your loved one in a different way than it does mine.There can also be a more activities during the holidays than are reasonable. For example, parties, shopping, baking, cleaning and decorating not to mention the added expenses. Yikes!!
Frequently we have unrealistic expectations for ourselves and our person living with Dementia. Let's consider whose needs are really being served by these traditions.
What do I really want or need?What does the Person Living with Dementia (PLwD) want or need?Should the goal be to just ENDURE the Holidays??I vote NO! Let's think outside the box. What can we change to serve both of our needs?
How can we choose wisely:Let's have a family meeting if there are other decision makers involved.
1.Call a family meeting to discuss which traditions are a must or what is open to change.
2. Do you include the PLwD in the meeting? That depends on their awareness.
3. Consider what we are willing to give up.
4. Ask for Help! Let go of your ego and save yourself.
5. At large events, assign a person to be one on one with your PLwD and tend to their needs. They can give them insight and serve them quickly before they get agitated.
6. They can also recognize when this person needs to leave the room with all the activity for a time out, potty break, snack or something else. Preventing a larger reaction is always good plan.
7. Put yourself first when it comes to your health habits.
8. Take time to to step away for a break to breathe and recharge your physical batteries.
It's important to share this information about your person with friends and family so they will have realistic expectations, but the sad truth is not everyone will understand what you are going through. We can always do a zoom call with A Better Approach to Memory Care so they can hear explanations from me. It is so important to have them on the same page pulling in the same direction with you.
Care Partners need to acknowledge their feelings. Please reach out to family, community, social, spiritual and homecare support as needed.As families grow and change, traditions will too. Choose a few to hold on to and let go of the rest. Maybe go to online visits and exchanging pics instead of gifts to save money. These little changes can provide huge relief for you and your loved one during the Holiday Season.

Although most of us plan to always stay in our own homes, statistically that will not be the way things go. You need to be proactive and make a plan for if and when your situation changes to assure you are prepared financially, legally, and with your personal choices in mind.
In this podcast we will review the signs of physical and mental decline which will indicate that if we continue to live we will eventually decline and need more help. Types of help might be homecare, transportation, placement assistance or even care management.
Then there is the money part which works better with great planning. In America we are blessed with a medicaid safety net if needed but most people would like to be in control when possible. This brings us back to the title of this podcast, Seniors Need to Know Their Plan B.

Ep 28: Dementia Care Tips for First Responders

If we start with some pictures of possible emergency scenarios that you might encounter, many of you can immediately explain what you do next. There is a car on fire, a man holding his chest or the very pregnant woman who appears to be in great distress. Your training and experience will direct you on what to do next just like mine did when I worked in the level three Trauma Center in my twenties. I don't claim to know your job but I knew what to do once you brought them to me at the Trauma Center.
The challenge comes with the picture of the woman who doesn't seem to have any problems at all. She appears to be a sweet elderly woman with a grandbaby on her lap, but let's look a little closer. Can you see the Dementia? No? Well she actually is mid stage with her journey with dementia and can become very demanding if you push the wrong buttons. That is where we struggle. We need to be better detectives with skills to join them on the journey and make sure we are not part of the problem when working with these individuals.
Next, we will review some of the more common symptoms of Dementia and how you can respond. Probably the most important tip I can give you today is to be aware of Anosognosia. This is a term that changed everything about my care for my Mom and I need to credit Teepa Snow with Positive Approach to Care for this education. Anosognosia is the Inability for a person to see their own brain deficit. We described how it affects people living with BiPolar Disorder or Schizophrenia but the greatest revelation for me was learning it can develop in 50% of people living with a Dementia.
It is important to understand that some people living with dementia have awareness of their diagnosis and are usually sad, tearful and maybe even fearful about their disease. Others with Anosognosia have no awareness of this problem and as a matter of fact it is always your problem. They are paranoid, suspicious, never wrong, always right but always the victim. These can be a no win situation without training and this is when they call for help from First Responders or from me to sort out why they do what they do.
We are dealing with a person with no logic so no amount of explaining will suffice. Hit the Pause Button and realize that what works is to Validate their position, De-Escalate the energy in the room THEN we can redirect them emotionally or physically to a happy place. We need to allow them to think they are right even when they are wrong and ask them to help us if possible to get them to do something. Sounds counterintuitive but it works.
Better Ways to Communicate When You Recognize Anosognosia:
Always approach from the front, imagine they are wearing a scuba mask as most people with Dementia lose peripheral vision.
Don't Rush them and use fewer words.
Extend a Hand and Ask them for Help.

Ep 27: Dementia Care and Council on Aging

COA is a vital organization serving the five counties in SW Ohio. They strive to serve older adults so they can stay in their homes as long as desired. Some of the services include meals delivered to the home and or transportation. Caregivers also get a lot of support with COA which may include a variety of safety equipment.
There are also programs called Elderly Services which is a taxpayer funded program that can fill in the gap when someone is not low income enough for medicaid, however they are struggling to afford the needed care. Then there is a service called Passport for individuals who do qualify for medicaid and would be forced prematurely into nursing homes without it.
COA is a non-profit and wants to provide the right amount of care, at the right time for the appropriate needs.
Income does not determine eligibility for care so do not assume you are ineligible for services. There may be co-pays or sliding scale costs to make the services available.
If you are looking for care in another part of the country you can use the eldercare locator. www.eldercare.acl.gov. This works on a zip code basis.
Interesting fact: about 70% of Americans will probably need long term care but only 30% believe they will need it!! Huge discrepancy and COA hopes to keep the masses at home as desired.
Caring for the caregiver is vitally important and the shortage of personal caregivers is at an all time low and many families are being thrust into providing personal care for their family members. Caregiver burnout frequently is the reason people end up in nursing homes too. COA is working hard to provide what is needed to keep a balance at home whether they are local or at a distance.

Ep 26: Dementia Care: How to Keep the Peace

Caring for a loved one with memory loss can be one of the most difficult tasks you will face in your life. We haven't been trained for this type of care and frequently find ourselves arguing and fighting about things that really shouldn't matter but they seem to escalate into situations that can soon damage our relationship.
Thankfully there are more techniques now on how to keep the peace with a loved one struggling with dementia and we will share some here today. Keep in mind that maintaining a great relationship with this person is a priority. If they lose trust in us they may never want to even be in our presence and this is a very sad place to be in.
We need to join them on the journey not as their boss or superior but as their partner in life on the same team. Can we make them feel like we are figuring things out together? I vote yes. Avoid making the mistake of taking everything from them and insisting on doing it yourself because they are too slow. Ouch! That won't win you any points and it sure won't help grow this relationship.
Try to make them feel like you are just as forgetful or have the same challenge figuring out things like the new remote for the TV. You might see how you can provide tools to help them remember. For example, get a large calendar or provide notes to remind them to take their medication. There is a season where this will work and then we will help in different ways when they decline.
Always keep in mind that they tend to remember how you make them feel. They rarely remember every word you use but frequently remember how you make them feel. With this in mind let's avoid rushing, correcting and over explaining and see what we can do to bring them joy in the moment. Share a compliment or a story from the past where they were awarded or successful!
One more suggestion is to not play good cop/bad cop with individuals struggling with memory loss when it is time to implement hard choices. What if we blamed a 3rd party like the doctor for not being able to drive or the insurance company for requiring home care. We can still validate their frustration arm in arm on the journey and let our tone and body language reaffirm that we are still on their team. Make a choice to be part of the solution not part of the problem!! You Got This!

the brain is an amazing and very complex part of the human central nervous system which also includes the spinal cord. it controls thinking, memory, language, hearing, learning, vision, balance and even our breathing and heart rate. it is hard to imagine all that the brain controls and monitors until something starts to malfunction.
neurodegenerative disease is an umbrella term for the huge range disorders which primarily affect the neurons or brain cells. there is so much still to learn about how the brain works and can it replace or repair damaged neurons or not. some popular examples of neurodegenerative diseases include alzheimer's, parkinson's etc.
symptoms of these diseases are many and some you may have experienced, but they can also be indicative of other disorders. let's compare the symptoms of some of the more popular diseases: alzheimer's, huntington's, parkinson's and amyotrophic lateral sclerosis. some symptoms overlap yet others are specific to certain areas of the brain experiencing damage due to disease.
when we dig a little deeper into the warning signs, the lists are long because people are unique and can be very different physically, culturally, and involving a variety of comorbidities. this complicates the ability to find an accurate diagnosis and sometimes our loved one will not tolerate further testing. then we do not and should not force this and frequently end up with the goal of comfort care and symptom management since we are unable to specifically diagnose our person.
when we compare and contrast alzheimer's vs parkinson's we can immediately see that alzheimer's frequently starts with cognitive decline and parkinson's frequently starts with physical challenges. this is interesting but confirms that with different areas of the brain declining, symptoms can be very different.

Ep 24: Bizarre Activities You Might See with Dementia

There are so many examples of interesting to very bizarre activities I have seen over the years. Today I will share a few of these examples with the hope of letting you relax and hit the pause button when it happens to you. Sometimes they are hoarding and hiding items with the hopes of making sense of it later or maybe paranoia has set in and we need to block the doors or make a plan to escape.
The important thing to remember is that your situation with your loved one or the person you care for may be different but frequently we can still accept and agree or clean up afterwards to avoid confrontations or corrections which can damage the relationship. The relationship is still so very important, and my advice is to make a plan to bring in help before the need is critical.

She received her Ph.D. in neurobiology from the University of Chicago and has thrived at multiple universities over the past two decades, most recently Longwood University in Virginia, where she earned tenure and launched an interdisciplinary neuroscience program. Her collaborative research over the past two decades has appeared in prestigious journals including Nature and Brain Research.
Catherine is widely recognized for her ability to take the latest research in neuroscience and psychology and apply it to everyday life, decision-making, and business strategies. Her thoughts and voice appear in hundreds of publications including CNN and Scientific American. Her broad scientific training combined with her innovative thinking, humor, and passion for informal education lends a unique lens to a range of topics, from holiday spending to the love of pumpkin spice.

Early in my experience with dementia I saw people with memory loss labeled pretty quickly. I even had a gentleman tell me, “I guess they all get mean and then you have to put them somewhere.” This statement really breaks my heart because there’s so much more to these individuals who are suffering with a disease they never asked for. If it was a broken hip, would you ask them to take the steps? Would you make it that difficult? Teepa Snow says if you’ve met one person with dementia, you’ve met one person. This is so true because each of these individuals are different, and their needs will be different. The techniques that worked for my mom may or may not work for your loved one, but we can investigate and come alongside and join them on the journey. Having the ability to pause, and seek to understand, “Why they do what they do”, is vital.

Once her Mom was showing symptoms she became a Certified Dementia Practitioner then went to North Carolina to train under Teepa Snow with Positive Approach to Care (PAC). She is still a Certified Independent Trainer with PAC. At A Better Approach to Memory Care, Teresa can be hired as a Speaker, Staff Trainer and Family Consultant. He Coordinates Their Community Paramedic Program And Peer Support Team. He Has Been A Member Of The Tri State Peer Support Team Since 2019. He Is Currently Working On His Masters At Xavier University To Become A Licensed Practice Counselor. He Is A Certified Ohio EMT-P, EMS/Fire Instructor, State Fire Inspector, IAFF Peer Support 16 Hour. Jim Is Passionate In Supporting Others To Live A Physically, Mentally, And Spiritual Quality Of Life.

Law-enforcement officers walk into danger every day. Sometimes there may only be a split second recognize whether this person has mental illness or dementia. Training is necessary to recognize these deficits in an individual. Join us as we listen to one amazing, retired law-enforcement officer and his experience with dementia and autism training and awareness.
A Better Understanding LLC with Tim Sutton
Tim Sutton is a retired 34 year veteran Law Enforcement Officer. After spending 4 years as a
patrolman for the Chesterfield County Police Department VA, he was hired by the Hanover County
Sheriff’s Office in Virginia. Tim then worked as a patrol deputy for two years and then spent four years
as a Street Crimes Investigator. He was promoted to the rank of Sergeant in 1999 and performed as a
patrol Sergeant until January of 2023. In 2003, he implemented a tracking system for his agency that
addressed the issue of individuals wandering or eloping due to cognitive impairment. He holds a
Bachelor of Business Administration Degree from James Madison University.
Tim formed A Better Understanding LLC (www.tim-sutton.com) in 2011 to raise awareness
and trained public safety officers on Alzheimer’s Disease, Autism Spectrum Disorders, and other
disabilities. Tim has been deemed a subject matter expert by the Virginia Department of Criminal Justice
Services and The International Association of Chiefs of Police on Alzheimer’s as it relates to Law
Enforcement. The Virginia DCJS has also declared him a subject matter expert in the area of
Autism/Intellectual Disability as they relate to law enforcement. He has been certified by the Circuit
Court of Hanover County VA as a Subject Matter Expert in the area of Autism as it relates to Law
Enforcement. Tim’s goal with A Better Understanding LLC is to promote Peaceful Interactions for
Positive Outcomes.
In 2022, Tim began podcast which can been seen and heard on all major platforms. A New
Perspective with Tim Sutton brings attention to those in the Disability (Differ’ability) Community who
are doing amazing things. The show is to educate, entertain, and inform viewers on the amazing abilities
of those with Differ’abilities.
Tim is also vice president of a nonprofit named A Better Understanding
(www.abetterunderstanding.org). ABU’s mission is to provide community events for our friends with
Differ’abilities. ABU educates, and raises awareness for individuals and companies who are making a
difference in our community.
Tim lives in Central Virginia and has three amazing adult children (a son and twin girls)

Our job is to come alongside and join them on their journey without lots of questions, expectations or demands.
I will review the great points from Better Communication Loss Part 1 then move into why we need to stay on your loved ones team. I will share a few stories to bring home the points on better ways to share information even when they may be having hallucinations or other worries that seem unfounded.
